The sustentees whose names are on the 'List of Sustentees 2022' must fill in the 'Sustentation Application Form' as soon as possible and send the completed form as well as a photo to Mrs. Glennes Anis.

1. Download the 'Sustentation Application Form'

2. Fill in your information

3. Attach your photo with the application form

4. Send your completed form to:

         b. Hand deliver to Mrs. Glennes Anis at CPC Ela Beach office.
